Privacy Act Information
The New Federal Privacy Act requires us to inform you about information which we gather on you. Information which you provide to us is used to ensure proper identification and history taking. We require only necessary information to ensure appropriate clinical care is taken within this office. Information is confidential except for emergency situations or with proper authorization given by you (or a legal guardian) to release personal data to another individual. We have a policy book for our clients to review at any time should you wish further information on this Act and our policies.
